Nurse Practitioner - Infusion Center (PRN)

About the role

TwelveStone Infusion Centers LLC manages the infusion needs of patients, maintaining the continuum of care from intake, assessment, and treatment through the completion of their visit. The primary focus is the safe and effective administration of biologic medications (e.g., Remicade, Tysabri), along with a full range of infusion services including antibiotics, hydration, vitamins, iron, steroids, sterile dressing changes, and lab draws. This PRN Float Nurse Practitioner position covers Germantown, TN; Jackson, TN; Southaven, MS; and surrounding infusion centers.

The role is responsible for providing comprehensive care, including assessment, diagnosis, treatment, management, education, health promotion, and care coordination for patients and families with acute and chronic health needs in an outpatient setting.

Responsibilities

• Provides patient and family-centered IV infusion care.
• Facilitates consistent, coordinated care and clear communication among all members of the healthcare team.
• Incorporates the patient and family in care planning and treatment across settings.
• Serves as an advocate for patients and families in navigating the healthcare system.
• Communicates and collaborates effectively with physicians, health professionals, health-related agencies, patients, and families.
• Works effectively with multidisciplinary team members and promotes mutual respect and trust within a team model.
• Reviews, updates, and recommends relevant medical, clinical, and administrative policies, procedures, and education to ensure high-quality patient care.
• Actively participates in quality initiatives.
• Maintains accurate and timely records and reports, and participates in patient service and program reviews.

Requirements

• Master’s degree in nursing from an accredited nursing school.
• Active and unrestricted Advanced Practice Registered Nurse (APRN) and Registered Nurse (RN) license in the state of practice, with current active board certification from ANCC or AANP. May substitute a degree and certification from NCCPA with an active, unrestricted license in the state of practice.
• CPR certification (BLS minimum, including hands-on/in-person assessment).
• 2 years of experience as an NP or PA in a clinical setting, or 3+ years as a Registered Nurse in oncology, ER, ICU, and/or infusion center. Infusion experience is required.
• Demonstrated high degree of clinical expertise in working with patients with acute and chronic illnesses.
