Nurse Practitioner – In-Home & Telehealth Care

Position Overview

ACE Physicians is expanding into Knoxville, Tennessee, and seeks compassionate and motivated Nurse Practitioners to provide high-quality, patient-centered care through in-home visits and telehealth.

Key Responsibilities

• Perform comprehensive health assessments and physical examinations during in-home visits and telehealth appointments
• Evaluate, diagnose, and treat acute and chronic conditions with an emphasis on preventive care
• Develop and implement individualized treatment plans tailored to each patient's needs
• Educate patients and families on health management, wellness, and disease prevention
• Collaborate with physicians, specialists, and interdisciplinary care teams to ensure coordinated care
• Perform minor clinical procedures such as wound care as appropriate during home visits
• Maintain accurate and timely documentation in electronic medical records (eClinicalWorks or similar platforms)
• Advocate for patients by providing respectful, inclusive, and equitable care, including individuals with disabilities

Qualifications

• Active, unrestricted Nurse Practitioner license (required)
• Experience in geriatrics, internal medicine, or primary care preferred
• Previous home health or home-based care experience strongly preferred
• Experience caring for patients with disabilities is highly desirable
• Comfortable delivering care in both in-home and telehealth settings
• Excellent communication skills and a collaborative, team-oriented mindset
