About the role

The Department of Family and Community Medicine is recruiting Advanced Practice Providers to join the Timothy Freeman, MD, Center for Developmental Disabilities within the UC Health system.

Responsibilities

  • Provide primary care to adults with a variety of intellectual and developmental disabilities including autism, cerebral palsy, Down syndrome, and many rarer syndromes - 60% of clinical time will be in the community setting through home visit model and 40% of clinical time will be on-site at the Freeman Center.
  • Collaborate with interdisciplinary team.
  • Communicate well with patients with a variety of communication styles, caregivers, families, support staff, and multiple specialists within the health system to coordinate care.
  • Document findings, plan, and actions taken according to departmental guidelines and standards.
  • Maintain a working knowledge of relevant medical/legal issues that impact on patient care.
  • Maintain records and statistics in accordance with department policies.
  • Attend and participate in staff, committee, department, and other administrative meetings.
  • Participate in department's continuous quality improvement efforts.
  • Provide education to other health care professionals.
  • Perform other duties as assigned.

Requirements

  • Master’s Degree in Nursing from an accredited nursing program.
  • Active Ohio APRN license with prescriptive authority.
  • Active Board Certification in appropriate nursing specialty.
  • Three (3) years nursing experience.
