Nurse Practitioner I (IKC)

DaVita Integrated Kidney Care (DaVita IKC) is an integrated care division of DaVita Inc., working towards the vision of providing integrated care to all End Stage Renal Disease (ESRD) patients. With more than 600 dedicated nurses, care coordinators, nurse practitioners (NPs), and business professionals, DaVita IKC manages care for over 20,000 patients with late-stage chronic kidney disease (CKD) and ESRD each month. Responsibilities
- Manage and coordinate care for patients with chronic kidney disease and end stage renal disease.
- Collaborate with interdisciplinary teams to ensure comprehensive patient care plans.
- Provide clinical guidance and support to patients and their families.
- Ensure adherence to regulatory standards and best practices in kidney care.
- Participate in performance improvement initiatives to enhance patient outcomes.
Requirements
- Bachelor's degree in Nursing, Healthcare Administration, or related field.
- Current licensure as a Registered Nurse (RN) in the state of practice.
- Experience in managing patients with chronic kidney disease and end stage renal disease.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
Qualifications
- Minimum of 2 years of relevant experience in renal care or related field.
- Knowledge of renal physiology and pathophysiology.
- Experience with electronic health records (EHR).
- Excellent problem-solving and decision-making skills.
Skills
- Strong clinical judgment and decision-making skills.
- Effective communication and interpersonal skills.
- Proficiency in Microsoft Office Suite.
- Ability to work in a fast-paced, dynamic environment.

Pay

Schedule
This position will be open for a minimum of three days. The schedule will be determined based on the needs of the department and the individual's qualifications.
