Nurse Practitioner Hospice PRN

About the role

Providing palliative, symptom management, and supportive care to hospice patients.

Responsibilities

• Conduct face-to-face visits with hospice patients and report findings to the Hospice Medical Director
• Collaborate with physicians, initiate and coordinate care plans, and document patient progress
• Instruct and supervise patients and families in self-care techniques
• Maintain accurate clinical notes and communicate patient needs to the team

Qualifications

• Active unrestricted license and current certification as a Nurse Practitioner or Clinical Nurse Specialist
• At least 5 years of nursing experience
• Valid driver’s license and access to reliable transportation

Skills

Preferred skills include experience with interdisciplinary teams, clinical care in oncology, hospice, geriatrics, or intensive care, and palliative care certification.
