About the role
The Hospice Nurse Practitioner provides palliative, symptom management, and supportive care consultations to hospice patients, collaborating with physicians and the interdisciplinary team to develop and coordinate individualized care plans.
Responsibilities
- Conduct face-to-face patient visits within prescribed timeframes and report findings to the Hospice Medical Director.
- Consult with physicians, inform primary caregivers, and collaborate with the care team to deliver comprehensive hospice care.
- Develop, implement, and document care plans, goals, interventions, and patient/family responses.
- Educate and supervise patients and families in self-care techniques.
- Maintain accurate clinical documentation and communicate patient status and needs to relevant team members.
Qualifications
- Active unrestricted license and current certification as a Nurse Practitioner or Clinical Nurse Specialist.
- Minimum of 5 years of nursing experience.
- Current driver's license, vehicle insurance, or access to transportation.
Skills
- CPR certification (preferred).
- Experience with interdisciplinary teams (preferred).
- Oncology, geriatrics, or hospice care experience (preferred).
- Palliative care certification (preferred).

Pay
Competitive hourly pay range of $46.00 to $69.00, dependent on experience and location.
