The Nurse Practitioner will provide comprehensive, patient-centered care in both Hospice and Palliative Care settings, supporting patients with serious illnesses across various environments.

Responsibilities

• Perform patient assessments
• Manage symptoms
• Prescribe treatments
• Order diagnostics
• Facilitate goals-of-care discussions
• Collaborate with interdisciplinary teams to ensure high-quality, compliant care
• Complete Medicare-required visits such as hospice Face-to-Face encounters
• Maintain detailed documentation
• Support care transitions

Qualifications

• MSN or higher
• Active Nurse Practitioner and RN licenses
• National certification
• Prescriptive authority
