About the Role

The Nurse Practitioner will serve in a dual Hospice and Palliative Care role, delivering comprehensive, patient-centered care across serious illness settings.

Responsibilities

• Perform various visit types such as Face-to-Face encounters, assessments, symptom management, and goals of care discussions
• Ensure compliance with CMS and regulatory standards
• Collaborate with interdisciplinary teams and support care planning
• Prescribe medications within legal scope
• Coordinate care across settings like homes and facilities
• Complete documentation and Medicare recertification
• Facilitate transitions in care levels

Qualifications

• MSN or higher
• Active Nurse Practitioner license
• National certification
• Experience in hospice or palliative care preferred
• Strong clinical assessment and communication skills
• Knowledge of billing, regulations, and quality measures
