About the role

As one of Daymark’s Nurse Practitioners, you’ll work with our care team to implement our cancer care model with patients in their homes. Your primary goal is to provide hands-on, compassionate patient care directly in patients’ homes and through virtual platforms.

Responsibilities

  • Provide clinical care: Conduct assessments, place orders, prescribe medications, document appropriately, and assign tasks to enable our broader care team to deliver great patient care.
  • Enable Daymark’s ability to triage for high acuity: Navigate and provide direct clinical care for our highest acuity patients while supporting our broader patient population.
  • Collaborate with oncology practices in our market through daily standups with their clinical leadership teams.
  • Support Patients Across the Full Care Continuum: Provide comprehensive clinical support from active treatment through survivorship or end-of-life care, adapting your approach to meet each patient where they are.
  • Build Trusting Relationships: Establish strong, trusting relationships with patients and their care networks by prioritizing empathy and active listening in every interaction.
  • Real-world application of a new care model: Collaborate with Daymark’s clinical operations team to refine and implement our care model. Deliver the right care at the right time to patients with a cancer diagnosis.

Requirements

  • You are an active NP or PA and have been practicing for at least 4 years, delivering direct clinical care to patients.
  • Oncology, home care, palliative care, hospice, or primary care experience required.
  • Oncology Nursing Society certification, experience with value-based care and/or in home care are each a plus.

Organizational & Technology Skills

  • You excel at multitasking and thrive in fast-paced environments while maintaining meticulous organization in communications and documentation.
  • You’re comfortable using Slack, Zoom, Google Suite, and EHR systems.

  • You can roll up your sleeves and contribute to clinical problem solving.

  • You know how to identify and prioritize your initiatives and ensure that urgent and important tasks always get done.

Comfort with Ambiguity

  • You have experience being a part of building or launching new programs and are comfortable with changing processes to improve patient care.

Player/Coach

  • You can lead your team and can care for patients and switch contexts with ease, as all will be required in a day’s work.

Able to Work in-person

  • You'll deliver care to patients in Los Angeles, CA.

Availability

  • You can work with our team and patients full-time, four days per week.

Additional Information

The expected compensation for this role is $140,000 to $170,000 annually, depending on location & experience. This is a full time role operating on a 4 day / 10 hour schedule, Monday - Friday. All Daymark Health patient-facing roles require Basic Life Support (BLS) certification and compliance with Daymark's Employee Vaccination and Testing policy. All roles that require in-home visits to patients must provide proof of a valid driver's license.
