About the role
We are seeking compassionate and driven Nurse Practitioners to join our dynamic Clover Home Care teams in Gloucester County, NJ. This is a unique opportunity to provide comprehensive primary and supportive care directly in the homes of some of our most vulnerable members, helping to improve health outcomes and quality of life.
Responsibilities
- Conduct comprehensive in-home health assessments for enrolled Medicare Advantage members.
- Perform post-discharge follow-up visits to reduce hospital readmissions.
- Collaborate with interdisciplinary teams using data-driven clinical tools.
Requirements
- Ideal for Nurse Practitioners who are proactive, skilled in transitional care, and confident using technology and clinical data to guide care.
Qualifications
- You are located in or near Gloucester County, New Jersey and able to travel locally for home visits.
- You have an active and unencumbered NJ Registered Nurse license.
- You are a Certified Advanced Practice Nurse Practitioner (APN).
- You have a master’s degree or post-master’s certificate in advanced clinical practice.
- You have a minimum of 1 year of NP experience in a clinical setting.
- You have active DEA and CDS registration.
- You have current Basic Life Support (BLS) certification.
- You have experience in home care or house call settings (preferred, but not required).
Skills

Pay
A reasonable estimate of the base salary range for this role is: $140,000 - $165,000 USD
Schedule
Full-time employees accrue up to 18 days of vacation per year. Clover recognizes 10 federal holidays, including New Year's Day, Juneteenth, and Christmas. There are also 4 additional paid holidays, and 1 floating holiday.
