Nurse Practitioner – Geriatric Acute & Primary Care | $200K–$250K + Full Benefits
Role and responsibilities
We are seeking an experienced Nurse Practitioner to care for a geriatric and medically complex patient population in Brooklyn, NY. This is a blended primary and acute care role spanning multi-comorbidity management, home hospital medicine, and hands-on wound care. The right candidate brings substantial hospital medicine experience, is comfortable managing high-acuity patients, and is proficient in wound assessment, treatment, and debridement. This is a full-time (part-time option available for the right candidate), fully onsite position with an immediate start.
About the Practice
The practice delivers continuous, close-contact care for older adults and patients with multiple chronic conditions — spanning primary care, acute/home hospital medicine, and specialized wound management.
What You'll Do
- Manage a geriatric, multi-comorbidity panel across chronic and acute care
- Deliver acute care services, including home hospital medicine, for patients needing higher-acuity management outside the traditional inpatient setting
- Perform comprehensive wound care: assessment, treatment planning, ongoing management, and debridement
- Complete H&Ps, order/interpret diagnostics, and build and adjust treatment plans
- Prescribe and manage medications, including controlled substances
- Cook up with the clinical team, specialists, and families for safe transitions and continuity of care
- Maintain accurate, timely documentation in line with Medicare and payer requirements
Non-Negotiables
- Active NY State NP license
- Current DEA registration
- Active Medicare credentialing/enrollment
- Hospital medicine experience
- Wound care experience
Required Qualifications
- Board certification: FNP, AGPCNP, or AGACNP (acute care/hospitalist background strongly preferred)
- 5+ years of experience (Hospital Medicine and Critical Care preferred)
- 1–2 years of dedicated wound care experience

Note: Applicants with a minimum of 2 years of NP experience are encouraged to apply if they bring several years of geriatric and wound care experience as an RN — we value strong clinical backgrounds built across both roles.
