Job Summary

The Nurse Practitioner will facilitate Reserve Care's efforts to relieve suffering, enhance comfort, promote quality of life and foster choice in the presence of an advanced serious illness for all patients and families. The practice is based on competent clinical knowledge and focuses on clinical practice, education, collaboration, consultation and research related to palliative care.

What You Will Do

Clinical Practice

• Participates in development and implementation of new programs that affect clinical practice
• Performs holistic patient assessment
• Prescribes for symptom management and/or recommends symptom management to primary care providers
• Performs medical history and physical exam in collaboration with the physician to develop a plan of care
• Carefully coordinates care with team members and health care professionals involved in patient and family plan of care
• Reviews medical record including physician/consultant notes, lab and diagnostic test results. Formulates plan of care based on above data
• Follows up to reassess the plan of care as determined by patient’s goals
• Participates in analyzing and evaluating clinical practice standards in terms of cost and quality
• Participates in development and implementation of new standards of practice for hospice and palliative care

Consultation

• Ensures quality care across the continuum as an expert in palliative care
• Serves as a consultant to staff caring for patients in hospitals, long-term care settings and home settings
• Serves as an expert consultant to healthcare professionals at the local, national and international levels to influence and advance clinical practice in palliative care

Education

• Maintains a thorough knowledge of the current practice and literature relating to palliative care and facilitates the sharing of that knowledge with staff
• Edifies patients and families on applicable areas of palliative care, such as interventions for pain and symptom management, and choice in end-of-life care
• Empowers patients and families to identify and meet goals of care
• Edifies RC staff in areas of palliative care
• Edifies outside agencies, hospitals and long-term care settings on principles of palliative care

Collaboration

• Maintains a standard care agreement with RC Medical Directors
• Works as part of a team
• Collaborates with members of the palliative care team
• Collaborates with health care professionals as appropriate within the community
• Facilitates communication between nursing staff/physicians and patient/family
• Participates on RC committees
• Participates in quality improvement activities
• Maintains and cultivates professional relationships in healthcare that strengthen RC’s strategic marketing plan
• Fulfills professional growth through involvement in professional organizations, presentations and publications

Research

• Participates in agency research endeavors
• Presents findings of research internally and at professional conferences

Accountability

• Participates in peer reviews
• Adjusts to changes in workload and schedules based on changing departmental and organizational priorities
• Maintains national certification
• Sets palliative care educational opportunities to increase knowledge
• Teaches and lectures for continuing education programs
• Serves as a preceptor to Advanced Practice Nurses

Conditions Of Employment

• Compliance to Annual Flu Shot Policy or ability to provide exemption documentation
• Provide an active auto insurance policy as a licensed driver in the State of Ohio with the proper level of coverage as directed
• Provide an active driver’s license

Physical Requirements And Working Conditions

• This is medium to heavy work requiring the exertion of up to 75 pounds of force occasionally and up to 20 pounds of force frequently, and the use of appropriate safety transfer techniques and body mechanics when moving, transporting, etc. of patients, sometimes in excess of 100 pounds; work requires stooping, reaching, standing, walking, lifting, fingering, grasping, and feeling; vocal communication; hearing; visual acuity and color perception.
