Nurse Practitioner for In-home Visits
About the role
Conduct in-home health evaluations for Medicare Advantage, Medicaid, and certain commercial plan members. These visits focus on a person’s overall health and well-being and identify potential barriers to care.
You will perform a medication and medical history review and a physical evaluation in the member’s home. Each evaluation uses a pre-populated, patient-specific form; you will not prescribe medicine, order lab tests, or alter the member’s current treatment regimen.
Responsibilities
- Conduct 6–9 in-home visits per day on a pre-scheduled basis
- Review medication and medical history
- Perform physical evaluations
- Document findings using provided patient-specific forms
Requirements
- Board-certified Nurse Practitioner
- Active state license (Medicaid license or willingness to enroll with our support strongly preferred)
- Independent Contractor (1099) status
- Flexible availability Monday–Sunday (no nights or weekends required)
Pay
- Base rate of $80 per completed assessment
- Earn up to $2,500–$4,000 per week
- Per-visit rate can increase up to 135% with Market Multiplier*
- Additional compensation via pay-per-test model for qualifying visits
- Availability incentives based on weekly availability
