What You’ll Do

• Deliver in-home acute and high-acuity care
• Perform comprehensive health assessments, and diagnose and treat mild to complex illnesses
• Analyze test data to diagnose, treat and arrange appropriate follow-up for the patient to ensure continuity of care
• Provide procedures including suturing, splinting, wound care, and IV therapy
• Order and interpret point-of-care diagnostics and imaging
• Educate patients and families on treatment and follow-up care
• Navigate goals-of-care and end-of-life conversations as needed
• Document accurately and adhere to clinical standards

What You’ll Need

• Current, unrestricted NP or PA license in the state of practice
• Minimum of two years of experience as an APP in the Emergency Department, Urgent Care, Internal Medicine, Hospital, Family Practice, Cardiac, Pulmonary or other relevant areas
• National board certification (current)
• Current BLS / ACLS certification
• Prescriptive authority and DEA (or ability to obtain within 60 days of hire; assistance provided)
• Valid driver’s license with clean driving record
• Availability for evenings, weekends, and holidays (expectation to work 3 holidays annually)

• Regularly required to sit, stand, walk, reach with hands and arms, and to talk and hear
• Occasionally lift, push and/or move up to 50 pounds
• Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception and ability to adjust focus
• Able to drive and sit in a car for long periods of time
• Able to drive during the day and at night
• Able to climb three flights of stairs while carrying equipment
• Able to push, pull, and lift 50 pounds
• Able to visually interpret test results
• Able to hear with or without accommodation
• Able to provide patient care in non-traditional or smaller spaces with limited room for movement
