What You’ll Do
- Deliver in-home acute and high-acuity care
- Perform comprehensive health assessments, and diagnose and treat mild to complex illnesses
- Analyze test data to diagnose, treat and arrange appropriate follow-up for the patient to ensure continuity of care
- Provide procedures including suturing, splinting, wound care, and IV therapy
- Order and interpret point-of-care diagnostics and imaging
- Educate patients and families on treatment and follow-up care
- Navigate goals-of-care and end-of-life conversations as needed
- Document accurately and adhere to clinical standards
What You’ll Need
- Current, unrestricted NP or PA license in the state of practice
- Minimum of two years of experience as an APP in the Emergency Department, Urgent Care, Internal Medicine, Hospital, Family Practice, Cardiac, Pulmonary or other relevant areas
- National board certification (current)
- Current BLS / ACLS certification
- Prescriptive authority and DEA (or ability to obtain within 60 days of hire; assistance provided)
- Valid driver’s license with clean driving record
- Availability for evenings, weekends, and holidays (expectation to work 3 holidays annually)
Essential Functions
- Must be able to perform the essential functions of the role, with or without reasonable accommodation
- Regularly required to sit, stand, walk, reach with hands and arms, and to talk and hear
- Occasionally lift, push and/or move up to 50 pounds
- Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception and ability to adjust focus
- Able to drive and sit in a car for long periods of time
- Able to drive during the day and at night
- Able to climb three flights of stairs while carrying equipment
- Able to push, pull, and lift 50 pounds
- Able to visually interpret test results
- Able to hear with or without accommodation
- Able to provide patient care in non-traditional or smaller spaces with limited room for movement
