Nurse Practitioner - Fee for Service, $5,000 Sign On Bonus

About the role

TruHealth, a division of American Health Partners, provides personalized, coordinated health care to aging patients primarily in nursing homes and assisted living communities. As an advanced practice provider (NP or PA), you will exercise independent judgment to treat acute and chronic conditions, consult with supervising physicians as needed, and work closely with facility partners and health plan leadership to implement Treat in Place Protocols in compliance with federal and state regulations. TruHealth owns nursing homes, health plans for Medicare beneficiaries in long‑term care, and offers psychiatric care, home health, hospice, rehabilitation, and specialty pharmacy services.

Responsibilities

• Assess patients’ medical and mental health needs and provide a plan of care that proactively manages conditions and addresses barriers to care
• Ensure compliance with local, state, and federal agencies related to clinical services provided
• Prescribe medications and order lab work, diagnostic procedures, and consultations
• Monitor patients’ compliance and response to treatment and modify plans accordingly
• May be required to provide training and advice to facility staff (supervisory responsibility)

Requirements

• Current APRN or PA license
• Degree from an accredited APRN or PA program
• Minimum 3 years’ experience in clinical nursing or rehab with geriatric populations, or Fee for Service experience
• Electronic Health Records (EHR) experience
• Working knowledge of Microsoft applications (Word, Outlook, Excel)

Qualifications

• Preferred education: Bachelor’s degree or higher; Master’s degree or higher
• Experience in primary care or internal medicine, preferably with a geriatrics focus

• Ability to work collaboratively with providers, case managers, facility partners, and health plan leadership
• Independent judgment and decision‑making for acute and chronic conditions
