Nurse Practitioner - Fee for Service, $5,000 Sign On Bonus

Position Summary

TruHealth is seeking an advanced practice provider—either a nurse practitioner (NP) or physician assistant (PA)—with experience in primary care or internal medicine, preferably with a geriatrics focus. The role involves providing personalized, coordinated health care to residents of nursing homes and assisted living communities, improving patient outcomes and preventing rehospitalization. You will exercise independent judgment to treat acute and chronic conditions, consult with a supervising physician as needed, and work closely with facility partners and health plan leadership to implement Treat‑in‑Place protocols in compliance with federal and state regulations.

Responsibilities

• Assess patients’ medical and mental health needs and develop proactive care plans that address conditions and barriers to care.
• Ensure compliance with local, state, and federal regulations related to the clinical services you provide.
• Prescribe medications and order laboratory work, diagnostic procedures, and consultations.
• Monitor patients’ adherence to treatment, evaluate response, and modify care plans as necessary.
• Collaborate with a multidisciplinary team of providers and case managers to improve outcomes for long‑term‑care residents.
• May provide training and advice to facility staff as required.

Requirements

• Current APRN or PA license.
• Degree from an accredited APRN or PA program.
• Minimum three years of clinical experience in nursing, rehabilitation, or geriatric populations, and/or fee‑for‑service experience.
• Experience with electronic health records.
• Proficiency with Microsoft Word, Outlook, and Excel.
