Responsibilities

  • Patient assessment and teaching and learning needs are accurately documented, detailed nursing history is obtained, and patients at physiologic and or psychological risk are identified.
  • Diagnosis is appropriately formulated with pertinent therapeutic and diagnostic procedures ordered.
  • Treatment plan is implemented with consults and referrals with interdisciplinary team.
  • Plan of care is reassessed, revised and modified based on physiological and psychosocial findings.
  • Plan of care is discussed with patient and family.
  • Education is provided to patients based upon identified risk factors.
  • All patient care activities are accurately documented.
  • Multidisciplinary teams are utilized appropriately.
  • Communication facilitated to maximize patient outcomes.
  • Identifies, provides, develops and maintains learning resources for patient care team.
  • Patient documentation is accurate and legible.
  • Safe therapeutic care is provided in holistic and systematic way.
  • Trends in patient populations are recognized and alternatives to care are explored.
  • Participation in clinical research and review of current nursing research literature.
  • Assistance in the development and revision of standards for patient populations.
  • Other duties as assigned are completed within specified time frames.

Qualifications

  • Advanced Practice License from Louisiana State Board of Nursing and Certification as Nurse Practitioner by ANCC or AANP.
  • Current Louisiana state RN license.
  • Current BLS Certification (within 6 months of hire).
