Responsibilities
- Patient assessment and teaching and learning needs are accurately documented, detailed nursing history is obtained, and patients at physiologic and or psychological risk are identified.
- Diagnosis is appropriately formulated with pertinent therapeutic and diagnostic procedures ordered.
- Treatment plan is implemented with consults and referrals with interdisciplinary team.
- Plan of care is reassessed, revised and modified based on physiological and psychosocial findings.
- Plan of care is discussed with patient and family.
- Education is provided to patients based upon identified risk factors.
- All patient care activities are accurately documented.
- Multidisciplinary teams are utilized appropriately.
- Communication facilitated to maximize patient outcomes.
- Identifies, provides, develops and maintains learning resources for patient care team.
- Patient documentation is accurate and legible.
- Safe therapeutic care is provided in holistic and systematic way.
- Trends in patient populations are recognized and alternatives to care are explored.
- Participation in clinical research and review of current nursing research literature.
- Assistance in the development and revision of standards for patient populations.
- Other duties as assigned are completed within specified time frames.
Qualifications
- Advanced Practice License from Louisiana State Board of Nursing and Certification as Nurse Practitioner by ANCC or AANP.
- Current Louisiana state RN license.
- Current BLS Certification (within 6 months of hire).
