About the role
Community Health Network is seeking an Advanced Practice Provider to join their family medicine team at Family Medicine Care, Anderson.
Responsibilities
- Provide comprehensive primary care services to patients in a family medicine setting
- Collaborate with physicians and other healthcare professionals to develop and implement treatment plans
- Perform physical exams, order and interpret diagnostic tests, and provide appropriate referrals as needed
- Prescribe medications as necessary and maintain up-to-date knowledge of prescribing practices
- Engage in patient education and counseling regarding health maintenance, disease prevention, and lifestyle modifications
Requirements
- Must successfully complete a nationally recognized Advanced Practice Nurse certification examination or have current national certification
- Maintain certification on an ongoing basis
- Must be eligible for prescriptive authority under the criteria outlined by the Indiana State Board of Nursing
- Maintain eligibility for prescriptive authority on an ongoing basis
Qualifications
- Master's degree in nursing from an accredited program
- Current national certification in advanced practice nursing
- Eligibility for prescriptive authority in Indiana
