About the role
The Atlantic Health is seeking a Diabetes Nurse Practitioner to support Endocrine Services. The NP will provide specialized diabetes care to adult patients at both outpatient and inpatient settings.
Responsibilities
- Provide comprehensive diabetes care to patients
- Ensure high-quality, evidence-based practice
- Collaborate with a multidisciplinary team to develop and implement individualized treatment plans
- Educate and support patients in self-management of their conditions
- Contribute to the growth and advancement of the Endocrine program through quality improvement initiatives
- Engage in continuous learning and professional development
Qualifications
- Graduated from an accredited NP program
- Master’s degree required
- Certified Diabetes Counselor Education Specialist (CDCES), or ability to obtain a certification within 1 year of hire
- ACLS and BLS certification required
- PREFERRED: Minimum 1-2 years’ experience in acute care and/or physician practice setting with diabetes patients
- Excellent communication and computer skills
- Strong work ethic and a team player who can work independently
