About the Role
Cambridge Health Alliance (CHA) is seeking a full-time Clinical Specialist II, Nurse Practitioner, to join our CHA PACE Program. CHA PACE is a Program of All-Inclusive Care for the Elderly (PACE) serving adults 55 years and older with chronic medical and/or mental health difficulties. The goal is to achieve the highest level of community independence and prevent nursing home placement. The role involves working within a high-functioning interdisciplinary team to integrate and coordinate medical and behavioral health care, community resources, and in-home support services.
This position offers the opportunity to work in a unique healthcare model with a complex adult population. Nurse Practitioners are a vital and respected part of the practice, valued for their collaboration in a supportive environment.
Responsibilities
- Provide care at short-term rehab (STR) facilities or cover GoTo assignments as assigned.
- Work on-site daily, either at the PACE center in Cambridge or at an assigned STR center.
- During GoTo coverage at the PACE center, handle calls from home care nurses, nursing homes, triage nurses, clinic treatment nurses, and pharmacy, as well as occasional walk-ins and same-day sick visits.
- During STR coverage, follow patients and provide care within the facility.
Qualifications
- Graduate of an accredited Nurse Practitioner program with a Master’s degree in Nursing.
- Current NP certification and licensure in Massachusetts.
- MA RN Licensure required.
- Current Massachusetts licensure authorizing practice in an expanded role, including a DEA number and Mass DPH registration to prescribe controlled substances.
- Minimum of 2 years of experience in geriatric health as an NP preferred.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a collaborative team.
Pay
Salary range is $61–$111/hour based on years of experience.
Schedule
36 hours/week, Monday through Friday (MNA Cambridge Union 32/40).
