About the role

Community Healthcare Network, a Federally Qualified Health Center (FQHC) serving the underserved communities of Brooklyn, Bronx, Manhattan and Queens, is seeking a Family Nurse Practitioner for our East New York Health Center located at 999 Blake Avenue, Brooklyn, NY 11208-3535. Reporting to the Medical Director, this individual will be responsible for providing evidence-based Primary Care to our patients. This individual is an integral part of the collaborative care team which includes nursing staff and other services such as Health Education, Nutrition, Social Work and Behavioral Health.

Requirements

• Valid Family Nurse Practitioner N.Y.S. License to deliver medical care
• At least two (2) years of post-graduate/residency experience delivering care in a Primary Care/Ambulatory Care setting
