About the role
The Nurse Practitioner examines and treats patients independently and in autonomous collaboration with other health care professionals following established standards and practices. Ensures proper illness and injury care and disease prevention, diagnosis, treatment, and recovery. Relies on education, experience, professional training and judgement to accomplish responsibilities. A wide degree of creativity and latitude is expected. Works under minimal supervision under the direction of the collaborating physician, as appropriate. May lead and direct the work of others within scope of practice.
Responsibilities
- Patient Care: Accurately document patient assessment and teaching/learning needs; obtain detailed nursing history; identify patients at physiologic and/or psychological risk through physical assessments that identify priority care needs. Formulate diagnosis appropriately and order pertinent therapeutic and diagnostic procedures to implement plan of care based on care standards and/or protocols. Implement treatment plan with consults and referrals to interdisciplinary team regarding patient assessment findings. Reassess, revise, and modify plan of care based on physiological and psychosocial findings; document in the treatment plan. Review patient plan of care with interdisciplinary team. Order tests and consults as required by plan of care; review test results 100% of the time. Discuss plan of care with patient and family with documentation. Provide education to patients based upon identified risk factors; develop, revise, and evaluate patient education programs based on effectiveness of learning.
- Partnership and Collaboration: Accurately document all patient care activities per documentation standards, including age-appropriate health education and community resource utilization; evaluate outcomes and contribute to performance improvement initiatives through medical record review. Utilize multidisciplinary teams appropriately and incorporate interdisciplinary expertise to achieve patient outcomes 100% of the time. Precept students in accordance to specified guidelines. Efficiently coordinate patient care activities and facilitate communication to maximize patient outcomes and ensure continuity of care. As a primary resource to the patient care team, identify, provide, develop, and maintain learning resources for the team as requested.
- Quality: Ensure patient documentation is accurate and legible based on focused chart reviews 100% of the time. Provide safe therapeutic care in a holistic and systematic way to facilitate patients' achievement of desired outcomes. Recognize trends in patient populations, explore alternatives to care, and assist in revision of standards, guidelines, and/or protocols as needed. Participate in clinical research and review current nursing research literature with demonstrated application to clinical practice. Analyze practices that contribute to variance, draw conclusions from aggregate data, identify resources to assist in variance analysis, and assist with drawing conclusions from aggregate data to improve performance outcomes. Assist in development and revision of standards for patient populations incorporating an interdisciplinary approach and utilizing best practices and research findings.
- Other Duties as Assigned: Complete other duties as assigned within specified time frames. Actively participate in hospital committees and other quality improvement activities as requested by manager.
Qualifications
- Graduate of an approved professional school of nursing
- Completion of State Board of Nursing approved Nurse Practitioner course and licensed to practice professional nursing as a Nurse Practitioner
- Certified in the State of Mississippi by the Mississippi State Board of Nursing and the American Nurses Credentialing Center as a Nurse Practitioner
- Current BLS Certification
