About the role

We are seeking compassionate and driven Nurse Practitioners to join our dynamic Clover Home Care teams in Mercer, Middlesex, or Somerset County, NJ. This is a unique opportunity to provide comprehensive primary and supportive care directly in the homes of some of our most vulnerable members, helping to improve health outcomes and quality of life.

As a Clover Home Care Nurse Practitioner, you’ll join our In-Home Care (IHC) Team, which provides longitudinal primary care for members with complex and chronic conditions, focusing on continuity, quality, and outcomes. Clinicians on this team have a high degree of autonomy and play a central role in ongoing care planning.

Responsibilities

  • Manage a dedicated patient panel, delivering ongoing in-home primary care.
  • Care for members with chronic illnesses and advanced medical needs.
  • Participate in a shared on-call rotation to ensure continuous support.

Best suited for Nurse Practitioners who value clinical autonomy, excel in building lasting patient relationships, and are skilled in managing medically complex cases throughout the continuum of care.

Requirements

  • Located in or near Mercer, Middlesex, or Somerset County, New Jersey and able to travel locally for home visits.
  • Active and unencumbered NJ Registered Nurse license.
  • Certified Advanced Practice Nurse Practitioner (APN).
  • Master’s degree or post-master’s certificate in advanced clinical practice.
  • Minimum of 1 year of NP experience in a clinical setting.
  • Active DEA and CDS registration.
  • Current Basic Life Support (BLS) certification.
  • Experience in home care or house call settings (preferred, but not required).

Pay

A reasonable estimate of the base salary range for this role is $140,000—$165,000 USD. Final pay is based on several factors including but not limited to internal equity, market data, and the applicant’s education, work experience, and certifications.
