About the role
We are seeking a Nurse Practitioner to service patients in the Athens area. This is a full-time salary-based position, with the opportunity to receive a monthly bonus.
Responsibilities
- Evaluate high-risk patients with complex medical needs and develop individualized care plans.
- Carefully coordinate care activities with other members of the interdisciplinary team to ensure continuity of care and optimal patient outcomes.
- Communicate regularly with case managers, clinical coordinators, social workers, specialists, pharmacists, health coaches, and other team members to coordinate patient care.
- Participate in interdisciplinary team meetings to discuss patient progress, treatment plans, and care coordination efforts.
- Foster effective communication and collaboration to facilitate seamless patient management and interdisciplinary teamwork.
- Meet staff efficiency expectations while providing high-quality care to patients in various settings, including homes, facilities, telehealth, clinics, and specialties.
- Document care management activities accurately and efficiently to support patient care plans and regulatory compliance.
- Provide patients with education and resources related to their conditions, treatment plans, self-management strategies, and preventive care measures.
- Offer support and guidance to patients and their families to enhance their understanding of healthcare options and promote active participation in their care.
- Adhere to all relevant healthcare regulations, policies, and procedures, including HIPAA and Medicare guidelines.
- Monitor quality measures and gaps in care, identifying opportunities for improvement and implementing corrective actions as needed.
Qualifications
- Nurse Practitioner (NP), Physician Assistant (PA), or Physician with a valid license to practice in the state of employment required. License must be in good standing.
- Demonstrated experience in providing patient care, preferably in a home health, clinic, facility, telehealth, or specialty care setting.
- Strong communication, collaboration, and interpersonal skills, with the ability to work effectively within an interdisciplinary team.
- Ability to evaluate high-risk patients, develop care plans, and coordinate care activities to meet patient needs and improve outcomes.
- Commitment to delivering compassionate, patient-centered care and promoting a culture of excellence in healthcare delivery.
- Strong organizational and time management skills, with the ability to multitask and prioritize responsibilities effectively.
- Strong written and verbal skills.
- Basic computer knowledge.
- Able to manage and demonstrate effective leadership skills.
- Should demonstrate good interpersonal and communication skills under all conditions and circumstances.
- Able to foster a cooperative work environment.
- Team player with ability to manage multiple responsibilities and demonstrate sound judgment.
- Must be able to work flexible hours and travel between offices, facilities, etc.
- Must be a licensed driver with an automobile that is insured in accordance with state and/or organizational requirements and is in good working order.
