Qualifications
- Minimum of 2 years experience in hospice, home health, or community health within the last 5 years
- Ability to lift and transfer patients as required, including objects up to 20 pounds
- Specific vision abilities: close, distance, and focus adjustment
- Stand or walk for more than one hour per day
- Stand, walk, sit, stoop, kneel, and crouch as needed
- Requires completion of accredited Nurse Practitioner program
- State licensure is required
