Nurse Practitioner

Qualifications

• Minimum of 2 years experience in hospice, home health or community health within the last 5 years
• Able to lift and transfer patients as required, including objects up to 20 pounds
• Specific vision abilities: close, distance, and focus adjustment
• Stand or walk for more than one hour daily, stand, walk, sit, stoop, kneel, and crouch as needed

Skills

Collaborates with hospice employees and volunteers as part of an interdisciplinary team (IDT)

Responsibilities

• Records physical findings
• Formulates plan and prognosis based on patient’s condition
• Discusses cases with physicians and other health professionals to prepare comprehensive patient care plans
• Prescribes or recommends drugs or other forms of treatment
• Maintains and monitors patient progress and reports progress to supervising physician
