About the role
Provide primary care to elderly participants in a community-based care program under physician supervision.
Responsibilities
- Evaluate health complaints and develop/implement care plans
- Collaborate with an interdisciplinary team to coordinate care
- Conduct reassessments and assist with discharge planning
Requirements
- Licensed Registered Nurse (RN) in the state of practice
- Graduate of a professional nursing program
- Certified as a nurse practitioner by a recognized certifying organization
- At least one year of experience with the frail and elderly population
- Training or related experience relevant to the role
- Preferred qualifications: Geriatric Nurse Practitioner certification with prescriptive authority, two or more years of geriatric nurse practitioner experience
