Nurse Practitioner

About the role

Provides continuing, comprehensive health maintenance and medical care to the patients of Butler County Community Health Consortium with a specific focus on culturally competent health care for the underserved. Acts as an advocate for improving health care quality and access for all populations.

Responsibilities

• Provides primary medical care to the patient population.
• Consults with staff physicians in making decisions about patient care.
• Evaluates and treats patients in health centers and school-based settings, ensuring continuity of care.
• Provides nighttime, weekday, weekend, and holiday call coverage as part of the work schedule.
• Delivers patient and family education and counseling.
• May participate in the training of nurse practitioner students as requested.
• Maintains complete medical records, including history, physical exams, and telephone message summaries.
• Completes progress notes and other special patient forms.
• Handles correspondence, phone calls, and letters to patients and referring physicians.
• Checks and acts on lab reports and correspondence within 24 to 48 hours; addresses abnormal labs immediately.
• Participates in health collaborative and health improvement activities.
• Engages in committee activities, chart reviews, and Quality Improvement/Risk Management initiatives.
• Attends department and center meetings as assigned.

Requirements

• Master of Science degree in Nursing.
• Graduation from an approved nurse practitioner program.
• Licensed by the Ohio Board of Nursing as a Nurse Practitioner.
• Two years of experience working with indigent populations and community-based programs preferred.
• Bilingual (Spanish) preferred.

Physical Demands

• Frequently required to stand, walk, use hands, reach with hands and arms, and talk or hear.
• Occasionally required to sit, stoop, kneel, crouch, or crawl.
• Must regularly lift and/or move up to 25 pounds.
• Specific vision abilities include close, distance, peripheral, depth perception, and ability to adjust focus.

Work Environment

• Occasionally exposed to fumes, airborne particles, toxic or caustic chemicals, and risk of radiation.
• Moderate noise level in the work environment.
