Nurse Practitioner
About the role
Pride Health is hiring a Nurse Practitioner in SC for a contract to hire position. The role involves working between two Long-Term Care/Skilled Nursing Facility (LTC/SNF) facilities in Richland County, SC, supporting new building launches.
Responsibilities
- Provide specialized care to high-risk health plan members
- Work as part of an interdisciplinary team
- Conduct comprehensive assessments
- Manage acute and chronic medical and behavioral conditions
- Ensure accurate documentation, including ICD-10 coding
- Communicate and collaborate with the care team
- Facilitate advanced illness and care planning discussions
- Offer counseling and education to patients and caregivers
Requirements
- Nurse Practitioner or Physician Assistant
- NP Certified in one of the following: FNP, ANP, GNP
- Access to transportation
- 1+ year of experience (LTC, SNF, Geriatric preferred)

- No weekends
- No holidays required

$65-70/hour
Schedule
Monday-Friday (8am-5pm)
