About the role
Join a senior-focused primary care team as a Nurse Practitioner, providing comprehensive outpatient care in a collaborative, patient-centered environment.
Responsibilities
- Report to the Associate Medical Director and work onsite
- Diagnose and manage moderate conditions
- Maintain accurate documentation and medical records
- Coordinate referrals and collaborate with interdisciplinary teams for holistic patient care
Qualifications
- Graduate of an accredited Nurse Practitioner program (MSN or DNP)
- Current national certification (AANP or ANCC)
- Active, unrestricted NP license in South Carolina
- DEA license and Medicare/Medicaid credentials or ability to obtain
Preferred Skills and Experience
- 2+ years of NP experience, preferably in primary care or Medicare Advantage settings with geriatric patients
- Experience with Athena EMR
- Familiarity with Medicare guidelines and HEDIS quality indicators
Work Environment
Outpatient clinical setting with a standard schedule.
