Nurse Practitioner

Essential Functions

• Carry out comprehensive health assessments for P3 members across various settings.
• Facilitate transition of care visits to ensure clear understanding of discharge instructions and minimize avoidable hospital readmissions or emergency department visits.
• Conduct comprehensive geriatric assessments using clinical judgment to identify care gaps and recommend interventions.
• Triage and manage non-life or limb-threatening conditions under the guidance of an interdisciplinary team.
• Collaborate with the Regional Clinical Medical Director or designee for clinical decision-making support.
• Educate P3 team members and patients/families/caretakers on health maintenance, chronic disease management, medications, and preventive care.
• Use technology and evidence-based medicine to guide care planning and decision-making while ensuring patient privacy and adherence to HIPAA regulations.
• Participate in quality improvement initiatives, documentation audits, and other activities supporting clinical excellence and operational efficiency.
• Maintain compliance with required trainings, timely chart closures, chart queries, meeting attendance, and other requirements.
• Serve as a positive and collaborative leader and team member, modeling P3 Health Partners core values.

Qualifications

• Active and unencumbered Nebraska Nurse Practitioner License.
• Current DEA license.
• Minimum 1 year of NP experience in geriatrics setting.
• Experience in home care or house call settings (preferred).
• Experience in geriatric acute care.
• Able to perform out-patient procedures such as wound care, management of skin tears and lacerations, intravenous hydration, phlebotomy, urinary catheterization, and epistaxis management.
• Flexible, self-driven, collaborative, and innovative personality.
• Willingness to travel within the market for patient visits external to P3 facility.
