Responsibilities

  • Provide medical support and coordination to hospice patients and family members.
  • Provide medical expertise in pain and symptom management and evaluate terminal prognosis for patients, families, and staff.
  • Ensure care delivery aligns with established hospice and palliative care guidelines, standards, and best practices.
  • Regularly communicate and collaborate with VHS providers regarding new patient referrals.
  • Perform patient visits in private residences, long-term care facilities, assisted living facilities, hospitals, and other settings, including telehealth.
  • Collaborate with attending physicians regarding patient care and treatment, and support the coordination of hospice referrals.
  • Provide the Hospice Medical Director with the required face-to-face encounter documentation.
  • Perform other duties as assigned by the Medical Director.

Qualifications

  • Graduation from an accredited School of Nursing and accredited Nurse Practitioner Program.
  • Has a current DEA license.
  • Current nursing licensure in State and CPR certification.
  • Master's degree with a minimum of one (1) year Nurse Practitioner experience and a minimum of one (1) year hospice or palliative care experience.
  • Accreditation from an approved certifying body for advanced practice nursing as required by State.
  • Certification in a specialist area preferred.

Requirements

  • Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.

Pay

$140,000 - $160,000 per year
