Nurse Practitioner
About the Role
Longevity Health is seeking a Nurse Practitioner or Physician Assistant with a passion for working with seniors in long-term care settings. In this role, you'll manage a panel of enrolled members receiving nursing home care, supported by a dedicated clinical leadership team. You'll conduct comprehensive assessments, develop individualized care plans, participate in care team meetings, and maintain communication with members' families to ensure alignment with their goals and needs.
Responsibilities
- Provides collaborative primary care and services to a panel of members.
- Delivers ISNP Model of Care activities, develops advanced care planning, and executes a treat-in-place model to avoid unnecessary hospitalizations.
- Facilitates Health Plan clinical operations functions, including prior authorization, Part A and B services, medication reviews, HEDIS and STARs quality gap closure, and membership growth.
- Serves as a trusted advisor to members, loved ones, nursing home staff, and colleagues.
- Maintains effective communication and coordination with members’ PCPs, loved ones, and interdisciplinary care team members.
- Supports market goals, balances member caseloads, and promotes Longevity Health to stakeholders.
- Functions autonomously, establishes meaningful relationships, and ensures high-quality, member-centric care.
Requirements
- 6+ months of experience as a Nurse Practitioner or Physician Assistant.
- Master of Science in Nursing (Nurse Practitioner) or Master’s Degree (Physician Assistant) from a nationally accredited program.
- Experience in a nursing home environment is strongly desired.
- Computer/technology proficiency.
- Strong communication skills with physicians, members, and family members.
- Self-directed yet collaborative team player with excellent clinical, critical thinking, and organizational skills.
Qualifications
- State unrestricted license as an Advanced Practice Nurse (AANP, ANCC, or AACN) or Physician Assistant (NCCPA board certified).
- DEA License, active NPI, and Medicare provider number.
- Valid state driver's license with a good driving record and proof of automobile insurance.
