About the role

An Advanced Practice Provider (APP) is a licensed and board-certified nurse practitioner or physician’s assistant specifically assigned to Longevity Health Plan members to execute the Institutional Special Needs Plan Model of Care. APPs requiring regulatory oversight or supervision will be hired based on available supervisory or collaborative practice physicians and oversight processes in each state where Longevity Health Plan operates.

The APP is responsible for medically managing a panel of Longevity Health Plan enrolled nursing home level of care or institutional members in partnership with the primary care team, delivering the Longevity Health Plan Model of Care. The APP conducts comprehensive assessments, develops and delivers individualized plans of care, attends nursing facility care team meetings, communicates frequently with residents’ families or loved ones, and ensures the care team understands the member’s goals of care and needs.

The APP focuses on treating members in place, uses clinical practice guidelines to guide treatment decisions, and documents members’ health status, functional status, and diagnoses. Supported by an APP Clinical Manager, Market Director of Clinical Services, and local medical director, the APP receives education and guidance on best practices for member care.

Responsibilities

• Provides collaborative primary care and services to a panel of Longevity Health Plan members, treating in the most appropriate setting based on the member’s goals of care.
• Executes and delivers Longevity Health Plan Model of Care activities, including advanced care planning and embracing a "treating in place" model.
• Facilitates and administers Health Plan clinical operations functions, such as prior authorization, Part A and B services, medication reviews, HEDIS and STARs quality gap closure, and promoting membership growth.
• Serves as a trusted advisor to members, loved ones, and nursing home staff, advocating for members and educating care teams on member needs and early identification of changes in condition.
• Delivers effective and frequent communication and coordination with members’ primary care providers, loved ones, and care team members regarding care needs, goals of care, and treatment plans or changes.
• Supports overall market goals and targets, balancing member caseload demands with team support needs.
• Promotes Longevity Health Plan with members, loved ones, nursing facilities, and other stakeholders.
• Functions autonomously and is accountable for practice while establishing meaningful relationships in the nursing home environment.
• Prioritizes member-centric and customer service-oriented care, ensuring high-quality outcomes for Longevity Health Plan members.
• Performs other related duties as assigned.

Requirements

• State unrestricted licensed Advanced Practice Nurse with national board certification (AANP, ANCC, or AACN) as an Adult, Family, Geriatric, or Primary Care nurse practitioner OR state unrestricted licensed Physician’s Assistant with NCCPA board certification.
• For nurse practitioners: Acute Care certification accepted if the candidate has nursing home, geriatric, or hospice/palliative experience.
• For physician’s assistants: Nursing home, geriatric, or hospice/palliative experience required.
• DEA License required.
• Active National Provider Identifier (NPI) and active Medicare provider number.
• Eligibility to participate in the Medicare program as a provider and registered.
• Valid state driver’s license with a good driving record and proof of automobile insurance.
• Auto liability insurance with minimum limits of $100,000/$300,000 for bodily injury and $100,000 for property damage (combined single limit of $300,000). Proof of coverage required annually.

Qualifications

• Master of Science in Nursing as a Nurse Practitioner from a nationally accredited program OR Master’s Degree from a nationally accredited program for Physician’s Assistants.
• Minimum of 6 months of experience as a Nurse Practitioner or Physician’s Assistant AND experience with geriatric, hospice, or palliative care populations prior to becoming an NP/PA OR a licensed and board-certified new graduate NP/PA supervised in the LHP New Graduate Fellowship program for 6 months, successfully completing all competencies for independent practice.
• Experience working in the nursing home environment is strongly desired.
• Computer and technology proficiency required.
• Excellent communication skills to engage effectively with physicians, patients, and family members.
• Self-directed but functions well in a team environment.
• Strong clinical skills in assessment, diagnosis, treatment, and prescribing, specifically for the chronically ill population.
• Demonstrated critical thinking, organization, and prioritization skills.
