About the role
Join a growing healthcare team as an Advanced Practice Clinician within an integrated care program focused on high-risk patients in nursing homes, senior housing, and assisted living settings.
Responsibilities
- Conducting comprehensive assessments with medical history review, medication reconciliation, vital signs, and physical exams
- Supporting members during short-term rehabilitation episodes
- Accurate documentation of clinical encounters, including ICD-10 coding
- Collaborating with interdisciplinary care teams
- Leading advanced illness and care planning discussions
- Providing counseling and education to patients and caregivers
Qualifications
- Active certification (ANCC, AANP, NCCPA) and state licensure with prescriptive authority
- Unrestricted license, reliable transportation, and ability to navigate varied environments
- Ability to establish collaborative practice agreements as needed
Preferred experience
- Over one year in clinical settings like long-term care, geriatrics, or home health
- Experience managing patients with complex behavioral, social, or functional needs
- Proficiency with electronic health records and technology
