About the role

You will address the suffering of an aging population with dementia, Alzheimer’s disease, and other neurocognitive disorders in nursing homes and assisted living facilities.

Responsibilities

  • Travel daily to assigned facilities from your home
  • Provide initial evaluations, continuing therapy services, and cognitive testing
  • Create and maintain positive professional relationships with client facilities, patients, and their families
  • Document the need and extent of services for each patient

Requirements

  • Proof of Nurse Practitioner licensure
  • Good time management skills
  • Able to function autonomously with regional Area Manager support

Qualifications

  • Able to work as part of an interdisciplinary treatment team

Skills

  • Autonomy of private practice
  • Collaboration with corporate clinical leadership
  • Administrative support from in-house departments
