About the role
You will address the suffering of an aging population with dementia, Alzheimer’s disease, and other neurocognitive disorders in nursing homes and assisted living facilities.
Responsibilities
- Travel daily to assigned facilities from your home
- Provide initial evaluations, continuing therapy services, and cognitive testing
- Create and maintain positive professional relationships with client facilities, patients, and their families
- Document the need and extent of services for each patient
Requirements
- Proof of Nurse Practitioner licensure
- Good time management skills
- Able to function autonomously with regional Area Manager support
Qualifications
- Able to work as part of an interdisciplinary treatment team
Skills
- Autonomy of private practice
- Collaboration with corporate clinical leadership
- Administrative support from in-house departments
