Responsibilities
- Involves with direct patient physical assessment, education, and health planning
- Maintains continuity of care by regular patient follow-up
- Affords assistance in coordinating services with other disciplines and patients' insurance plans
- Advocates for patients in arranging medically necessary services
- Refers patients to specialty services when necessary
- Consults with collaborating physician and other members of the health care team to establish plan of care
- Participates in quality assurance process to assure optimal care delivery
- Provides comprehensive case management services that include emphasis on health promotion and disease prevention
- Marks patient outcome in accordance to the practice protocols
- Collaborates with staff nurses and nursing administrators with assessment and clinical decision in patient care planning
- Conducts education in service on practice issues
Requirements
- Minimum Qualifications: Graduated from approved Nurse Practitioner Program, Licensed as Nurse Practitioner in New York State, Basic CPR (BCLS) certification required, At least 3 years of clinical experience as a primary care Nurse Practitioner, Board/Specialty Certification preferred, Bilingual in English and Mandarin Chinese required
Compensation And Benefits
- Full-time annual base salary at $105,000 - $115,000, with additional productivity-based incentive pay
