About the role
At Carease Health - IL, we're committed to providing high-quality preventative care to residents in skilled nursing facilities. As a Nurse Practitioner (NP) or Physician Assistant (PA), you'll play a crucial role in building strong relationships with our facilities, maintaining a patient-centered approach, and prioritizing proactive measures for enhanced physical health and well-being.
Responsibilities
- Deliver high-quality preventative care to residents in skilled nursing facilities
- Combine on-site visits with remote work from home, supported by a dedicated scribe team for efficient documentation
- Build strong relationships with the facility and be a collaborative team player
- Maintain a patient-centered approach and prioritize proactive measures for enhanced physical health and well-being
Requirements
- Active NP or PA license (in the state of practice)
- 1+ year of experience in Geriatric care
- Documentation review
- Excellent communication, collaboration, and teamwork skills
