Responsibilities
- Provide high-quality preventative care to residents in skilled nursing facilities, including on-site visits and remote work from home
- Utilize Remote Patient Monitoring (RPM), Chronic Care Management (CCM), and Collaborative Care Management (CoCM) technologies to enhance patient care
- Maintain accurate and timely documentation, supported by a dedicated scribe team
- Build strong relationships with facility staff, patients, and family members
- Remain up-to-date with industry developments, best practices, and evidence-based guidelines
- Collaborate with other healthcare professionals to ensure seamless care transitions and optimal patient outcomes
Requirements
- Active NP or PA license (in the state of practice)
- 1+ year of experience in Geriatric care and Documentation review
- Excellent communication, collaboration, and teamwork skills
- Masters-level education in a relevant field
