This hybrid role combines on-site visits with remote work from home, supported by a dedicated scribe team for efficient documentation. Responsibilities

• Deliver high-quality preventative care to residents in skilled nursing facilities
• Perform on-site visits and remote work from home

• Build strong relationships with healthcare facilities

Requirements

• Active NP or PA license in the state of practice
• 1+ year of experience in geriatric care
• Experience with documentation review
• Excellent communication, collaboration, and teamwork skills
