Nurse Practitioner

About the role

Carease Health is seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) with a background in Geriatrics or Internal Medicine to deliver high-quality preventative care to residents in skilled nursing facilities.

Responsibilities

• Deliver high-quality preventative care to residents in skilled nursing facilities
• Build strong relationships with the facility
• Maintain a patient-centered approach
• Collaborate with the scribe team for efficient documentation
• Work in a hybrid role combining on-site visits with remote work from home

Requirements

• Active NP or PA license (in the state of practice)
• 1+ year of experience in: Geriatric care, Documentation review
• Excellent communication, collaboration, and teamwork skills
