Nurse Practitioner
About the Role
We are seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) with a background in Geriatrics or Internal Medicine to join our team. As a hybrid role, you will combine on-site visits with remote work from home, supported by a dedicated scribe team for efficient documentation. Success in this role depends on building strong relationships with the facility, maintaining a patient-centered approach, and being a collaborative team player. You will work closely with our clinical directors, regional managers, and supervisors to provide exceptional care and support.
Responsibilities
- Provide high-quality preventative care to residents in skilled nursing facilities
- Conduct on-site visits and remote work from home, supported by a dedicated scribe team
- Build strong relationships with facility staff and maintain a patient-centered approach
- Collaborate with our clinical team to ensure seamless care coordination
- Participate in quality improvement initiatives to reduce hospitalizations and improve patient outcomes
- Maintain accurate and timely documentation of patient care
Requirements
- Active NP or PA license (in the state of Pennsylvania)
- 1+ year of experience in:
- Geriatric care
- Documentation review
- Excellent communication, collaboration, and teamwork skills
