About the role
This is a part time position of 18 hours per week.
Responsibilities
- Providing high-quality preventative care to residents in skilled nursing facilities through on-site visits and remote work
- Building strong relationships with the facility staff and residents
- Maintaining a patient-centered approach and being a collaborative team player
- Documenting patient care using a dedicated scribe team
Requirements
- Active NP or PA license (in the state of practice)
- 1+ year of experience in Geriatric care and documentation review
- Excellent communication, collaboration, and teamwork skills
