Carease Health – NY, a nationwide preventative healthcare company, is seeking an experienced Nurse Practitioner (NP) to join our team in Rensselaer, NY. As a key member of our clinical team, you will deliver high-quality preventative care to residents in skilled nursing facilities, combining on-site visits with remote work from home. Responsibilities

• Provide high-quality preventative care to residents in skilled nursing facilities, utilizing remote patient monitoring, chronic care management, and collaborative care management approaches
• Conduct on-site visits and work closely with a dedicated scribe team for efficient documentation
• Build strong relationships with facilities and maintain a patient-centered approach
• Collaborate with other team members to ensure seamless care coordination
• Participate in quality improvement initiatives and contribute to enhancing patient outcomes

Requirements

• Active NP or PA license in the state of practice
• 1+ year of experience in Geriatric care and documentation review
• Excellent communication, collaboration, and teamwork skills
• Strong problem-solving and critical thinking abilities
