About the role
As a Nurse Practitioner (NP) or Physician Assistant (PA) with Carease Health - OH, you will work in a hybrid setting that combines on-site visits with remote work from home, supported by a dedicated scribe team for efficient documentation.
Key Responsibilities
- Provide high-quality preventative care to residents in skilled nursing facilities
- Build strong relationships with facility staff and maintain a patient-centered approach
- Collaborate with a dedicated scribe team for efficient documentation
- Maintain patient progress and adapt care plans as needed
- Communicate effectively with patients, families, and healthcare teams
Requirements
- Active NP or PA license (in the state of practice)
- 1+ year of experience in Geriatric care and Documentation review
- Excellent communication, collaboration, and teamwork skills
