Carease Health - IL is a nationwide preventative healthcare company focused on reducing hospitalizations. This role involves working on-site in skilled nursing facilities while also having the option to work remotely.

Responsibilities

• Provide high-quality preventative care to residents in skilled nursing facilities, combining on-site visits with remote work from home
• Utilize a patient-centered approach, building strong relationships with facilities and patients
• Collaborate with our dedicated scribe team for efficient documentation
• Stay up-to-date with industry developments and best practices through continuing education and training

Requirements

• Active NP or PA license in the state of practice
• 1+ year of experience in Geriatric care and documentation review
• Excellent communication, collaboration, and teamwork skills
