Nurse Practitioner

About the Role

Become a Full-Time Nurse Practitioner in a compassionate elder care team. This role involves providing primary care, performing physical assessments, developing care plans, evaluating participant complaints, and delivering treatment and education to seniors and caregivers.

Responsibilities

• Conducting comprehensive history and physical assessments for new and existing participants
• Coordinating care with interdisciplinary team members and primary care physicians
• Overseeing residents in skilled nursing facilities and ensuring quality care
• Participating in discharge planning and managing emergent and acute needs
• Providing medical oversight for residents in skilled nursing facilities
• Collaborating with care teams to develop and implement personalized care plans
• Participating in team-based patient care and discharge processes

Qualifications

• Licensed registered nurse (RN) in the state of employment
• Graduate of an accredited nursing program
• Certified as a nurse practitioner by a recognized certifying organization
• At least one year of experience working with the elderly population

Preferred Skills and Experience

• Geriatric Nurse Practitioner certification with prescriptive authority (preferred)
• Two or more years of geriatric nurse practitioner experience (desirable)
