Nurse Practitioner, Advanced Practice Provider (Spanish Speaking Required)

Role: Nurse Practitioner, Advanced Practice Provider Responsibilities Provide high-quality, equitable primary care for adult Medicare patients.Conduct office visits for routine and acute issues, including wellness visits and health assessments.Coordinate care with specialists, testing facilities, and other providers.Lead population health initiatives, ensuring preventive screenings and health metrics are met.Assist with phone triage, patient education, and outreach efforts.Participate in promotional activities and conduct home visits as needed. Collaborate with a multidisciplinary team including physicians, nurses, and support staff, reporting to the Center Medical Director. Qualifications MSN, DNP, or NP certification in Family, Adult-Gerontology, or Gerontological Nursing.Active NP license, DEA certification, and US work authorization.Preferred experience in primary care, geriatrics, or internal medicine, with knowledge of HCC, ICD-10 coding, and health risk assessments.Strong teamwork skills and a passion for population health.
