Nurse Practitioner, Advanced Practice Provider (Bilingual Spanish Preferred)

About the role

We are seeking a Nurse Practitioner to deliver high-quality, equitable primary care to adult Medicare patients in an outpatient setting. The role involves working within a collaborative care team alongside physicians, nurses, medical assistants, and support staff, focusing on primary care, care coordination, and population health management.

Responsibilities

• conducting office visits for routine and acute issues
• administering wellness visits and health assessments
• coordinating care with specialists
• educating patients
• participating in population health initiatives
• assisting with outreach and home visits

Qualifications

• Master’s or Doctorate in Nursing
• national certification in Family, Adult-Gerontology, or Gerontological Nurse Practitioner
• active state license
• DEA license
• US work authorization

Bilingual skills and primary care experience are preferred.
