Responsibilities
- Provide effective, equitable, value-based primary care to adult Medicare patients
- Conduct office visits for routine and acute issues
- Administer wellness visits and health assessments
- Coordinate care with other providers and specialists
- Lead population health initiatives
- Assist with phone triage and outreach
- Educate patients
- Participate in promotional activities
- Conduct home visits as needed
- Collaborate closely with the care team and report to the Center Medical Director
Qualifications
- Master’s or DNP degree
- National certification in Family, Adult-Gerontology, Adult, or Gerontological Nurse Practitioner
- Active, non-probationary state license
- Active DEA license
- US work authorization
Preferred Skills
- Primary care experience (internal medicine, geriatrics, family medicine)
- Familiarity with HCC, ICD-10 coding, and HRAs
- Teamwork orientation
- Passion for population health
- Bilingual abilities are a plus

Schedule
